  • Insurance and Billing informationGateway is contracted with most insurance companies; however, you may want to contact your insurance to find out if you need authorization prior to services. The facility files for reimbursement from the patient’s insurance company as a courtesy, and all services rendered are the financial obligation of the patient, or guarantor. We participate with most insurance companies to provide you with the most cost effective option for your procedure.

Estimated and actual charges

Our insurance and billing staff are committed to providing you with specific information, including a pre-registration and insurance call prior to your visit. During your insurance call, our staff will go over your benefits, demographic information, and estimated costs. Sometimes there are variances between our estimated costs and actual costs. The costs listed on your estimate letter are only an estimate based off your physician’s pre-procedure evaluation and actual charges will be determined by the documentation provided to Gateway Surgery Center by your physician once your procedure has been completed.

Workers Compensation Claims:

If you are being seen under Workers Compensation, we will need have all of your claim information when your procedure is scheduled. The items needed for your claim are: Worker’s Compensation company and their address and phone number, your date of injury, your claim number, and the adjusters name and phone number.

Payment, Financing and payment plans

Gateway offers several financing opportunities for payment. For your convenience, Gateway Surgery Center accepts Master Card, Visa, American Express, Discover, Personal Check (with a valid driver’s license), and cash.

We currently offer two financing company options, Care Credit and Prosper Healthcare Lending, as well as we do have in-house payment plans that are interest free and under 6 months from the date of service.

Gateway’s in-house payment plans are initiated during the patient’s pre-registration and insurance education call. We offer up to a 6-month payment plan, depending on the status and age of the outstanding account balance.
